In this Photoshop tutorial, you will learn how to add to a shape extra elements. Just follow the steps of this free Photoshop tutorial and you will see how easy is to edit a shape and add more elements to it.
First of all let's draw a shape. For this let's use an existing custom shape. Select the Custom Shape Tool and draw the shape you want to change. After you did this, you should change in the upper menu the "Shape layers" setting with the "Paths" setting and a little bit right make sure is selected the "Add to path area" option. Normally, it should be selected. You can see in my examples where the two options I mentioned are.

Once you selected the mentioned options go in the layers window and select the Vector Mask of your shape. Now add a new shape to your original shape. The new shape will be added to your old shape.

If you want to draw something inside your shape and not adding an existing shape to your old one, select the Pen Tool, check the 2 options in the upper menu, select the vector mask of your shape and start drawing.

You can see in my example that I added a rectangle shape with the Pen tool to my original shape.
